The only human outcome literature that is large enough to take seriously is **pharmaceutical choline alphoscerate at about 1,200 mg/day for months** in older adults with Alzheimer’s or vascular cognitive impairment: a 2003 Mexican placebo-controlled RCT (ADAS-Cog about 6 points better than placebo at 180 days), Italian add-on work with donepezil (ASCOMALVA), and a 2023 meta-analysis that is real but fragile (monotherapy MMSE I² = 98%). That is **moderate** evidence for a **small-to-moderate cognitive effect in diagnosed dementia at the drug dose**, not a cure, and not independently reproduced in a modern large Western outcomes RCT.
