Acarbose competitively inhibits intestinal alpha-glucosidases, delaying carbohydrate digestion and reducing the post-meal glucose rise. It acts mainly in the gut and has not been shown to slow human aging.

Mechanism evidence: human
Acarbose is a real, cheap, **prescription** α-glucosidase inhibitor with **strong** evidence that, taken **with meals** in **type 2 diabetes**, it cuts 1-hour postprandial glucose by about **30–75 mg/dL** depending on background therapy and lowers HbA1c by about **0.5–0.8 percentage points** (van de Laar meta-analysis −0.77%; UKPDS 44 ITT only −0.2% because so many people stopped). **STOP-NIDDM** showed a **25%** relative reduction in incident diabetes over 3.3 years in overweight IGT (NNT ~10); **ACE** showed an **18%** reduction over 5 years in Chinese CHD + IGT (NNT 41). That is **delay of a glycemic diagnosis while the tablet is taken** — washout narrowed the gap — **not** a human lifespan result. The **STOP-NIDDM CVD signal** (49% relative reduction; 47 events; MI HR 0.09) is **hypothesis-generating and not confirmed**. **ACE** — 6,522 people, 5 years, primary MACE — was **null** (HR **0.98**). UKPDS 44 found no difference in major clinical events. Cochrane 2018: no firm CV-mortality or CV-event benefit. US labeling still says there is no conclusive macrovascular-risk-reduction evidence.

Flatulence, diarrhea and abdominal discomfort are common and dose-related. It is contraindicated with inflammatory bowel disease, colonic ulceration, intestinal obstruction risk, cirrhosis or diabetic ketoacidosis. When hypoglycemia occurs with insulin or a secretagogue, treat with glucose rather than sucrose because acarbose delays sucrose digestion. Liver enzymes and renal function require clinical consideration.
