At the **studied dose** — berberine HCl **500 mg BID or TID (1.0–1.5 g/day) for 8–16 weeks**, mostly in Chinese adults with T2D or dyslipidemia — there is a **real, replicated human signal**: HbA1c down about **0.4–0.7%** vs placebo/lifestyle, LDL-C down about **0.4–0.5 mmol/L**, TG down about **0.3 mmol/L**. That signal survives several metas (Dong 2012, Guo 2021, Blais 2023, Wang 2024) and one proper multi-centre RCT (Zhang 2008). Certainty is **moderate for “some glycemic and lipid effect at gram-doses in those populations”** and **low** once you ask about durability past 3 months, non-Asian patients, or hard outcomes.
