Resveratrol is rapidly converted to glucuronide and sulfate metabolites, leaving very little circulating parent compound. Direct SIRT1 activation in humans remains unestablished; proposed metabolic and anti-inflammatory pathways are largely based on cell and animal work.

Short trials commonly report gastrointestinal symptoms and headache. Higher doses may affect platelet function and drug metabolism, so people taking anticoagulants, antiplatelet medicines or medicines with narrow therapeutic ranges should seek clinical guidance. Long-term safety and benefit are not established.
